As a Nurse Practitioner or Physician Assistant with our practice, you will play a pivotal role in our mission to provide exceptional healthcare services directly to patients in the comfort of their homes or apartments. You will collaborate with our interdisciplinary team of pharmacists, nurses, MAs, phlebotomists, clinical liaisons, utilizing the latest tools and technology to enhance patient outcomes and streamline provider workflows.
Our current team of Nurse Practitioners and Physician Assistants bring a wealth of knowledge in primary care/internal medicine/palliative/hospice/geriatric experience and have all contributed greatly to our continued success. They each have an interest in home-based primary care with the goal of caring for older adults with complex chronic conditions in their homes, group homes, and independent living. Your Role
- Deliver medical care in the patient's home. This involves driving or riding in your vehicle to the place of service.
- Perform comprehensive health assessments and diagnose and treat complex illnesses.
- Follow At Home Harmony's Coordinated Model of Care of Medical and Pharmacy.
- Use critical thinking skills and follow evidence-based standards of practice.
- Accurately and thoroughly document your patient encounter and ensure accuracy.
- Analyze test data to diagnose, treat and arrange appropriate follow up for the patient to ensure continuity of care.
- Educate patients and families on how to manage their health and well-being.
- Effectively navigate difficult conversations related to end-of-life issues and goals of care.
- Communicate effectively with patients, family, the medical power of attorney, other medical providers and all individuals involved in the patients care.
- Adhere to clinical and safety standards, protocols, and performance metrics.
- Provide care with compassion, empathy, and cultural competency.
- Maintain positive relationships with Care Concierge and At Home Harmony staff.
- Attend training sessions and clinical team meetings, as requested
- Serve as ambassadors for At Home Harmony and maintain professional demeanor.
- Display a flexible, adaptable attitude and proactive demeanor as our company continues to grow and thrive.
Experience and Qualifications:
- Minimum of three years of experience as an APP in Geriatrics, Family Practice, Internal Medicine, Cardiac, Pulmonary with a passion for working with the Geriatric population
- Experience in home-based care a plus but desire to learn is welcomed
- Experience in senior living communities is preferred
- Graduate with an advanced degree from an accredited institution
- Current unrestricted VA state license as a Nurse Practitioner or Physician Assistant
- Nationally board certified, current
- Current BLS required
- Prescriptive authority and active DEA license
- Valid drivers license with clean driving record
- Ability to lift and carry equipment up to 50 pounds
- Ability to walk up and down several flights of stairs easily
Job status: Full time (0.8 FTE will be considered also)

Clinical Director
Princess Anne Ambulatory Surgery Center (ASC) is seeking a talented Clinical Director to join its team. The ASC is a partnership between community surgeons and Sentara and includes two operating rooms and two procedure rooms. The Center is independently owned and operated. The Clinical Director is responsible for overseeing daily clinical operations including preoperative, postoperative, intraoperative and sterilization departments as well as management and mentoring of the clinical staff (~40 employees) and collaboration with the business office staff. Each clinical department has a manager who reports directly to the Clinical Director. The Director plays an active role in the research, planning and implementation of strategies that align with the Surgery Center'- s overall goals for growth and development.
We are seeking a Clinical Director with surgery center experience who is able to analyze data and help the team make clinical improvements based on this analysis. The successful candidate will be able to work independently and successfully plan and manage new projects for the Center.
The ASC is open Monday - Friday; the ASC is NOT open evenings or weekends and no call is required.
Job duties include but are not limited to:
- Manages surgery center staff to provide pre, intra and post-operative services. Ensures appropriate staffing at all times.
- Establishes employee performance standards and expectations. Mentors and coaches staff as needed. Participates in the interviewing and hiring process.
- Ensures that surgical programs and clinical services are in compliance with all government regulatory agencies and accrediting bodies.
- Establishes, implements and evaluates quality and standards of patient care.
- Develops protocols and procedures for all departments.
- Negotiates the purchase and inventory management of supplies and medications. Ensures adequate levels of all drugs, supplies, instrumentation and equipment are available.
- Attends all Medical Advisory and Total Quality Management meetings as required.
- Performs quality assurance and improvement activities. Identifies improvement opportunities and analyzes them to develop solutions; acts decisively to implement solutions.
- Participates in budgeting process and financial problem solving.
- Identifies ways to reduce cost without compromising quality.
Experience and Certification:
- Minimum 3 years of ambulatory surgical experience, required
- Minimum 2 years of leadership and managerial responsibility, required
- Associate or Bachelors degree in nursing from an accredited college or university, required
- Current RN and BCLS Licensure, required
- Certification in OR (CNOR) or PREOP (CAPA) or PACU (CPAN), preferred
- Experience with accreditation survey (AAAHC) process, preferred

Medical Assistant - Mohs
This is an exciting opportunity to join a growing and thriving Dermatology practice. Richmond Dermatology is seeking a Medical Assistant to assist our Mohs Micrographic Surgeon and Dermatologist in all surgical procedures as well as patient exams. The successful candidate will be empathetic, energetic, and compassionate. The Medical Assistant will help manage patient flow, assist with patient care, function as an integral part of the team, provide unparalleled customer service, and tackle all tasks with the utmost professionalism.
Applicants must be a team player and have the ability to multi-task. This position will be required to:
- Greet, escort and prepare patients for physician visits/procedures and update their medical histories in the EMR.
- Assist physician with patient exams and all surgical procedures.
- Obtain medical histories, take blood pressure, administer medications, remove sutures, bandage wounds, provide patient instructions, etc.
- Responsible for handling, cleaning, packaging, sterilizing and maintenance of instruments and operating room equipment used in surgical procedures while adhering to OSHA guidelines.
- Complete daily statistical data and provide yearly totals (e.g., infection rates).
- Act as laboratory assistant as necessary, assisting with cover-slipping of microscope slides.
- Stock and clean exam and procedure room tables.
- Address all phone messages in a timely manner following proper procedures.
- Obtain pre-authorizations and referrals as needed for medications, treatments and procedures.
- Adhere to procedures for processing and tracking pathological specimens and other diagnostic testing.
